1,25-Dihydroxyvitamin D is the most active metabolite of vitamin D and circulates at pmol levels, whereas the precursor 25OH Vit D circulates at nmol levels. 1,25-OH Vit D is formed upon ultraviolet exposure and two hydroxylations, in the liver and in kidneys (Szymczak and Pawliczak, 2015). To measure this highly lipophilic molecule the assay has to combine a selective immuno-extraction followed by quantitation with an immunoassay (Hollis and Horst, 2007).

Bioclinica Lab employs a manual competitive radioimmunoassay, performed after an immuno-extraction, designed for the measurement of 1,25OH Vit D in serum.
